Student Forms

• Class Drop Form must be signed by a parent or guardian.
• Two weeks’ is required in order to drop a class.
• All Tuition is computed on the date of the written notice, not the last class attended by the student.
• All family Tuition fees must be current in order for a student to drop a class.
• Dancers who enroll in a class and then decide to drop will receive a credit for unused tuition which may be used for future classes at QC Dance.
• Credits are student-specific and non-transferable.
